Pfizer Malaysia’s New PCV20 Vaccine Gets Green Light From MOH

Pfizer Malaysia has officially announced the approval of its 20-valent pneumococcal conjugate vaccine (PCV20) by the Ministry of Health (MOH). This milestone enables the vaccine to be used for individuals as young as 6 weeks up to adults of all ages, a significant expansion from previous coverage.

The approval comes at a critical time as Malaysia continues to tackle high rates of pneumococcal disease, a serious illness caused by the Streptococcus pneumoniae bacteria. This disease can lead to life-threatening infections such as pneumonia, meningitis, sepsis, and ear infections, particularly affecting young children, older adults, and people with chronic illnesses like diabetes and heart disease.

Why It Matters

In 2023 alone, pneumonia was the leading cause of death in Malaysia, responsible for more than 18,000 fatalities. As Malaysia’s population ages, with 1 in 5 citizens expected to be over 60 by 2030 the need for stronger, broader immunisation options is more pressing than ever.

“With PCV20, we’re offering broader protection against 20 serotypes of pneumococcus,” said Ms. Deborah Seifert, Cluster Lead for Pfizer in Malaysia, Indonesia, Singapore, and the Philippines. “It’s about protecting more people, across more life stages, with a vaccine backed by over two decades of innovation.”

A Next-Generation Vaccine

PCV20 builds on Pfizer’s earlier vaccines, PCV7 and PCV13 by adding 7 more serotypes (8, 10A, 11A, 12F, 15B, 22F, 33F). These strains are linked to antibiotic resistance and have been increasingly found in invasive cases. In short, PCV20 offers more comprehensive coverage for today’s evolving disease landscape.

“Pneumococcal disease is often under-recognised until it’s too late,” said Assoc. Prof. Dr. Pang Yong Kek, a Senior Consultant Respiratory Physician. “Many older adults end up in intensive care due to pneumonia and it’s a preventable trend if we act early.”

Why Children Are Especially Vulnerable

For young children, early protection is crucial.

“A child’s immune system is still developing, especially in the first few years,” said Prof. Datuk Dr. Zulkifli Ismail, Consultant Paediatrician and Paediatric Cardiologist. “Infections like pneumococcal meningitis or sepsis can lead to serious, lifelong consequences. Prevention through vaccination is the best way forward.”

Strengthening Malaysia’s Adult Immunisation Efforts

The introduction of PCV20 also signals an opportunity to improve adult vaccination rates in Malaysia an area still lacking awareness and uptake.

“With the rise of non-PCV13 serotypes like serotype 8, it’s time we adapted our immunisation efforts,” said Prof. Dr. Mark van der Linden, Director of the National Reference Center for Streptococci, Germany. “PCV20 helps close those gaps in protection.”

In Malaysia, experts like Prof. Dr. Zamberi Sekawi, President of the Malaysian Society of Infectious Diseases and Chemotherapy, are urging for stronger education and advocacy efforts to encourage adult vaccination, especially among high-risk groups.

Approved in 40+ Countries

PCV20 has already received approval in over 40 countries, including the United States, United Kingdom, Hong Kong, and Singapore. Its use is backed by extensive clinical studies demonstrating both safety and strong immune responses across different age groups even when administered alongside other routine vaccinations.

As the vaccine becomes more widely available in Malaysia, health experts are hopeful it will help reduce the burden of pneumococcal disease and save lives across all generations.
